Application Requirements

APPLICATION REQUIREMENTS FOR PHILIPPINE BASE TRAINING

Please submit with the application form:

1. P 500.00 (refundable deposit). After the evaluation of your application,
the balance will be collected.

2. One recent Passport or 2×2 size ID picture attached to the application form.

3. Endorsement letter from the Pastor.

REGISTRATION PROCEDURE

1. Get an application form from the Jesus Rev office, or you may
click here to download.

2. Submit the application form with all the requirements at the JREV OFFICE.

Options are:

- Personally submit it. The office is located at the Life Center,
39 Miami Cor. Columbia Streets, Cubao, Quezon City.
-Fax it. (02)995-8583
-Email it. naziritetraining@jesusrev.com

3. After the evaluation of your application, you will be notified through a
phone call to remind you of your balance payment.

4. The training is limited up to 70 delegates only. You can avail the promo
by registering & paying the complete registration fee on or before July 31.

5. You will be issued an acknowledgement receipt, keep it for future references.

NOTE: Payments may be made by bank deposit. Please fax a copy of the
deposit slip with your name and contact number.

1. Fill up the Nazirite Training Registration Form and Application Form.

*Application Form for International Delegates
*Waiver for Minor Delegates (If you are bringing your kids,
you need to sign and submit this form)

2. You may also request for the forms by emailing Sis. Jane Nicose

3. Mail the accomoplished forms with the payment (check or money order) to:
